We are proud to be a ‘Safe Surgery’ for everyone in our community. All are welcome.

Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter -reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.

In partnership with Doctors of the World UK, we will ensure that our practice offers a welcoming space for everyone who seeks to use our services. Ask reception for an interpreter if you find it difficult to communicate in English

Registering Online –

Did you know you can now easily and quickly register to join the practice and become a patient online, as long as you are in our catchment area

You need to visit FIND A GP ONLINE NHS, or click on https://gp-registration.nhs.uk/H82041

When you use the online registration service you are asked all the questions needed to successfully register you. The service uses data validation. This means that you can only submit your questionnaire when all the mandatory information has been entered correctly. When you have finished registering online, we will be sent an email which will capture the information you have provided. This email should contain all of the information needed for us to successfully process your registration with surgery, we aim to do this within a matter of days where ever possible.

Register In Practice –

You can register by completing a form available from reception or clicking on the form below. If you would like to register for on-line services at the point of your new registration then please bring with you some form of Photo ID.

You are registered with the Practice rather than an individual GP and may see any of the doctors. You are able to express a preference of practitioner, and whilst the Practice will try to comply, it might not always be possible.

New patients have the opportunity to have a ‘new registration patient check’. This is a consultation with a Practice Nurse or our Health Care Assistant for a simple health check. You will need to bring a sample of urine to this consultation.

If you are on any regular medication, please note that you will need to see the doctor for this to be prescribed by the Practice.

Download our New Patient Registration Form (Word document) and a GMS1 Form (PDF file) to register as a new patient. You must fill in both of these forms.

Change of name / address

If your name or address have changed please download our change of personal details form and bring it into the practice
