This is usually when you have applied for life insurance, or claiming insurance due to medical conditions. You should be contacted upon your claim by the company to give consent for us to share medical information. You do have the option to view the medical information before this is sent off to the insurance company. Your consent form will be attached to the questionnaire they send to us. If there is no consent, we will not complete and will return blank. We charge the insurance company for this, Depending on your agreement with them.
