Holiday Cancellation Letter

We do require a form from your insurance company for completion. 

You must complete as much information in relation to this including dates.

We can only complete these, if you have consulted at the practice/hospital in relation to this reason for cancellation. On occasions if you have been advised by secondary care to not leave the country, and the practice has not been made aware, we may ask you to contact the department that advised this for completion.
