HGV/Taxi Medicals

If you are requiring a medical examination for a HGV or Taxi license/renewal, please follow these steps

1. HGV : Complete form D47P. This you will need to obtain from the DVLA/Post Office.
2. Taxi Medicals : Obtain a form through the local council.
3. Ask a doctor to complete the medical examination report (form D4 for DVLA). You can ask an optometrist or an optician to fill in the vision assessment section if your doctor cannot complete the whole report. Please you this form

4. You will be booked and appointment to have a medical assessment with one of our clinicians.
5. We will require payment of £180. This must be paid before your appointment. Appointments are usually 30 minutes.
6. After the medical appointment we will copy the form to add to your medical records, and give the original back to you to return to the DVLA
