Adoption or Foster Medical

The company you have gone through for your adoption or foresting application will advise of any forms that your GP needs to complete. Sometimes this does include a medical exam. The form may come from yourself or the company.

Should the third party require medical information, they would write to us, in which we would contact you to book an appointment. Or you may have the responsibility to book the appointment yourself.
